Before your first appointment, you will want to review the information below as well as print out, read and sign the forms linked below. If you are unable to print the forms, or forget to bring them, please come 15 minutes early to complete them in the office.
There is plenty of street parking available. Parking on 26th Street and 27th Street is free. My office is located on the second floor.
Everything shared in our sessions is completely confidential, including the fact that you are a client. The only exception to this is when there is clear and imminent danger of hurting yourself or someone else, or in the case of child abuse or elder abuse.
My fees for services are $110 for a 50-minute session, payable at time of service. I accept cash and checks. I can also accept credit cards but I charge a $2 processing fee.
I do not take any insurance, however, I can provide an invoice which you can mail to your insurer. Because I am considered an “out-of-network” provider, it is a good idea to contact your insurer and ask about your mental health coverage. In addition, most insurers require a mental health diagnosis in order to approve the coverage.
When we schedule an appointment, I have reserved that time for you specifically. With that in mind, I require 24 hour notice for cancellation or you will be charged for the session.
